I have seen this complain quite a few times in the past, almost always by those who use Rhithmic. Best advice is to keep the Rhithmic app handy to be able to flatten from there when its craps out like this.

Had this happening before as well. I would put in an order with brackets, cancel it, then a single would pop back in. A few times I was gone and it hit. Only noticed when I was using a Mac.
